Compounded Semaglutide
One of the most common questions we have been hearing from patients and practitioners recently is “Do we compound semaglutide?”
Semaglutide is the active ingredient in brand name drugs Ozempic® and Wegovy®. Ozempic® is FDA-approved for type 2 diabetes, and Wegovy® is FDA-approved for chronic weight management.
These drugs are being prescribed to assist patients with losing weight. These brand name drugs are currently in shortage according to the Food and Drug Administration (FDA) and often hard to find.

We currently do not compound sterile semaglutide injectables. At Morgan Compounding Pharmacy, we only compound non-sterile drugs. We are now able to provide alternative forms of semaglutide with a doctor’s prescription.

Alternatives to Semaglutide for Addressing Weight Loss and Blood Sugar Levels

Many if not most overweight people are suffering from insulin resistance. When insulin levels remain high for prolonged periods, it leads to weight gain. Insulin is an anabolic hormone that builds tissue in fat and muscle cells. In order to lose weight, one must improve insulin sensitivity at the tissue level to reverse insulin resistance and normalize blood glucose levels.
Transdermal Metformin
Metformin is an FDA-approved prescription medication commonly used to address blood sugar levels in patients with type 2 diabetes and prediabetes.
When taken orally, metformin is known to commonly cause very uncomfortable side effects like nausea, vomiting, diarrhea, gas, bloating and loss of appetite. These symptoms are experienced by 1 in 3 patients taking metformin by mouth.

We compound metformin into a transdermal/topical cream. Topically applied metformin using a permeation enhancing cream is a potential alternative for patients who cannot tolerate the side effects from the oral dose. This allows the drug to be absorbed into the blood stream while bypassing the gastrointestinal tract, where most side effects occur.
Studies have shown the topical metformin dose to be 10% of the oral dose patients receive. For example, if you are taking 500 mg by mouth twice daily, the topical dose would be 50 mg applied to the skin twice daily. It is typically applied to the inner arm, inner thigh, or behind the knee.
We compound and dispense transdermal metformin in a Topi-Click dispenser. The Topi-Click is a metered dose device that dispenses 0.25 ml of volume per click.
